The IMSAFE checklist is a personal fitness self-assessment that pilots should perform before every flight. It evaluates six areas of physical and psychological readiness that directly affect a pilot’s ability to safely operate an aircraft.
The acronym stands for:
• I — Illness
• M — Medication
• S — Stress
• A — Alcohol
• F — Fatigue
• E — Emotion (or Eating, as reflected in current FAA guidance)
IMSAFE is part of the “Pilot” component of the PAVE checklist. It provides a structured way for pilots to honestly evaluate their own fitness to fly—something that is surprisingly difficult to do objectively, especially when external pressures are encouraging you to go.
Even mild illness can significantly degrade a pilot’s performance. Congestion, headaches, nausea, and general malaise all reduce cognitive function, reaction time, and the ability to manage workload. Sinus congestion is particularly problematic in aviation because pressure changes during climb and descent can cause severe pain or vertigo.
The key question is not “Can I function?” but “Can I function at the level required to safely handle an emergency?” Normal flight may feel manageable, but illness dramatically reduces your capacity to handle unexpected situations.
Many common medications—both prescription and over-the-counter—can impair pilot performance. Antihistamines, sleep aids, some pain medications, and many others can cause drowsiness, dizziness, or cognitive impairment. The FAA maintains strict guidance on which medications are acceptable for pilots, and 14 CFR §91.17(a)(3) prohibits acting as a crewmember while using any drug that affects safety.
Importantly, the underlying condition that requires medication may itself be disqualifying for flight, even if the medication is not on the prohibited list. When in doubt, consult an Aviation Medical Examiner (AME) before flying.
Stress—whether from work, relationships, finances, or other life events—occupies cognitive bandwidth that would otherwise be available for flying. Acute stress narrows attention, impairs judgment, and increases the likelihood of errors.
Pilots under significant stress are more likely to:
• Miss checklist items
• Overlook changing weather conditions
• Make hasty decisions without full evaluation
• Become fixated on one problem while ignoring others
The FAA’s Aviation Instructor’s Handbook identifies stress as a significant factor in pilot error. The honest self-assessment required by IMSAFE is especially important here—most pilots can identify when they’re physically ill, but recognizing the effects of stress on their own performance is much harder.
Federal Aviation Regulations are clear on alcohol and flying. 14 CFR §91.17 states that no person may act as a crewmember of a civil aircraft:
• Within 8 hours after consuming any alcoholic beverage
• While under the influence of alcohol
• While having a blood alcohol concentration of 0.04% or greater
However, the “8-hour” rule is a minimum, not a guideline for fitness. Alcohol’s impairing effects—including degraded judgment, slower reaction times, and impaired coordination—can persist well beyond 8 hours, particularly after heavy consumption. Many safety experts and experienced pilots use a personal minimum of 12 to 24 hours from “bottle to throttle.”
Fatigue is one of the most underestimated hazards in general aviation. Unlike airline operations, which have duty-time and rest regulations, Part 91 operations have no regulatory limits on pilot duty time. The responsibility falls entirely on the pilot.
Fatigue degrades nearly every cognitive function required for safe flight:
• Attention and vigilance
• Decision-making quality
• Reaction time
• Memory and recall
• Spatial orientation
• Communication accuracy
The FAA’s Risk Management Handbook notes that the effects of fatigue are similar to alcohol impairment—and just as insidious. Fatigued pilots often believe they are performing normally when objective measures show significant degradation. Strong emotions—anger, grief, anxiety, excitement—can impair judgment just as effectively as physical illness. A pilot who has just received upsetting news or is anxious about a personal situation may not be in the right mental state to fly safely.
Current FAA guidance also includes “Eating” as part of this category. Hypoglycemia (low blood sugar) from skipping meals can cause symptoms similar to hypoxia: confusion, irritability, impaired judgment, and reduced coordination. Pilots should ensure adequate nutrition and hydration, particularly before longer flights.
The broader point of the E category is self-awareness. Am I in the right mental and physical state to handle not just a normal flight, but an abnormal situation?
